Social prescribing enables all local agencies to refer people to a link worker. Link workers give people time and focus on what matters to the person as identified through shared decision making or personalised care and support planning. They connect people to community groups and agencies for practical and emotional support.
Link workers collaborate with local partners to support community groups to be accessible and sustainable and help people to start new groups.
In north east London, we're making good progress. There is a well established programme for adults and plans developed for babies, children and young people, and ongoing projects capturing digital, evaluation, new pathways and workforce development.
A Community Chest is being set-up to address voluntary and community sector commissioning gaps identified through engagement with the social prescribing workforce. We are:
- Building communities of practice, provide support with quality and evaluation of support and outcomes
- Identifying strong local borough practice and helping to spread best practice across other boroughs
- Starting up a 'greenfield' approach, making good use of relevant external innovation and best practice
- Securing and embedding the use of common assets – data and digital solutions, training – and shared services
