Digital support for personalised care in north east London

Effective digital solutions are an integral part of personalisation, to:

  • enhance people’s choice and control over their health and care
  • ensure people’s needs are met
  • enable and accelerate the spread of personalised care.

Data captured by the workforce can provide commissioners and health and care practitioners with vital insights into the outcomes for local populations to help them identify unmet need and address health inequalities. The data tells us where the greatest need is, what it looks like and whether we have the capacity to meet it.

In north east London, our work on digital solutions focuses on:

  • safe and effective record capture and keeping
  • providing data to underpin reporting, communication and quality improvement
  • giving people the means to self-manage.

During 2022, the north east London personalised care programme has been working with the Evaluation Group led by Dr Victoria Tzortziou-Brown to refine the social prescribing template and dashboard. We have also been supporting teams in Newham, and Barking and Dagenham to procure a new social prescribing case management system.

The NEL social prescribing template collects a minimum dataset and has been rolled out in EMIS and SystmOne. It includes:

  • patient/resident demographic
  • presenting issue
  • intervention
  • patient recorded outcome measure data.

Data from the template is aggregated across NEL and is published daily in a PowerBI dashboard.

The dashboard enables insight into social prescribing activity across north east London, helping teams to identify local demand. The data is helping primary care networks to meet the updated PCN DES requirement to undertake proactive case finding for social prescribing. It also helps them easily to submit their social prescribing activity reports. The data is also starting to present a picture that can help shape services for the future and help inform commissioning decisions, for example through Community Chests.
